In light of the issue with the man made hole in the Soyuz at the ISS a month or so ago, it will be interesting to see what caused this malfunction. I expect the media to connect the dots and start asking about possible sabotage.

Well, they really can't say that hole up there is "accidental" now. Somebody doesn't want us up there anymore.

The Soyuz vehicle is about as safe as it can be but rockets even after all these decades are still prone to failure. However, these two events coming so close together is a bit strange, coincidence?

Two astronauts made an emergency landing following a rocket failure on a mission to the International Space Station.

Russia has launched a criminal investigation over a failed rocket launch to the International Space Station, according to reports.

A US and Russian astronaut were forced to make an emergency landing shortly after the mission got underway, with their Soyuz rocket having suffered significant engine failures.

It was an unprecedented mishap for the Russian space programme and the AFP news agency claims a criminal investigation is now underway to determine whether safety regulations had been violated during construction.

Unprecedented mishap? Not sure the Sky News folks did their research on that statement. Certainly the Russians have had a fair number of mishaps in their space program, although some were kept quiet during/from the Soviet era. The escape tower system that saved the crew today certainly has been used successfully before, a year or two before the Challenger blew up I remember its use when a rocket exploded on the launch pad. I also remember some talking head bringing up that accident when the Challenger exploded, and asking why we didn't use the same system. Idiot.
