Two of the stories are open to facts/interpretation. Thomas Mantell died because he chose to pursue the UFO, later identified as a classified Skyhook balloon, above 20K ft without supplemental oxygen. He passed out, and the Mustang spiraled down until it crashed. He died on impact.

The Cuban MiG-21 story has long fascinated me. I've read several accounts on the incident, including one from a respected Latin American aviation journal. Based on what I've read, I think the Cuban pilot got overly aggressive in his pursuit of whatever he was chasing and overstressed the airframe. Exceeding the structural design limits of an aircraft can result it it tearing itself apart. One of the saddest mishap investigations I ever supported was that of a young, aggressive USAF pilot who literally tore the wings off his aircraft while participating in dissimilar air combat training (DACT.)
